1. Purchase NetObjects Fusion MX from March 12, 2001 through September 30, 2001.
    
2. Print this page (or the PDF to the left) and complete the form below. Make sure you provide all mailing and contact information and sign the form. Sorry, P.O. Boxes cannot be accepted for mailing addresses.
    
3. If you have not already done so, complete the NetObjects Fusion MX registration process on the NetObjects Web site.  The registration card can be found inside the package, if you would like to mail in the registration. You must be a registered NetObjects Fusion MX user to qualify for the rebate.
    
4. Attach an original invoice or receipt to this form, with the NetObjects Fusion MX purchase circled. The store name, NetObjects product name, purchase price, and date of purchase must be clear. (Please keep a copy of your purchase receipt for your records.)
    
5. Tape or staple to this form the proof-of-purchase UPC bar code from the bottom of the NetObjects Fusion MX box.
    
6. Supply the title page, or the inside cover page, from your NetObjects Fusion MX User Manual.
    
7. Supply on this form both your NetObjects Fusion MX serial number and your previous NetObjects Fusion serial number.

Mail this completed form, the NetObjects Fusion MX registration card (if you have not already registered), the proof-of-purchase UPC bar code, the invoice or cash register receipt from your NetObjects Fusion MX purchase, and the manual title page from NetObjects Fusion MX to the following address:

Limit one rebate per registered copy of NetObjects Fusion MX.
